Cloud based secuserve works rather well if you don t care the servers are in europe. Barracuda as well but it is both expensive and a pita to configure. Eset is rather decent. Symmantec produced unacceptable filtering quality last time i checked. A good ol' postfix with screen and spamassassin would produce decent results but requires some mild tweaking and is a resource hog but will likely do fine unless you receive really huge flow
I'm going to second Proofpoint. It's cost effective, performs well and I've had very little issues with it. The Emergency mailbox feature is really good in the event of an actual issue. Users are still able to send and receive mail via the Proofpoint web portal even with the server being offline.
Personally I lean towards hosted email filtering opposed to filtering onsite. The main reason is because it's being filtered before it hits your environment.

an external provider is to your email what cloudflare and the likes are for a web site. any single server on a single connection can be trivially and sometimes accidentally DOSed... that does not impact any of your other services when using external providers, and even your email will handle loads of traffic you cannot expect to handle on premises. moreover if you are in the US given the crazily low, shared, and expensive bandwidths you are provided with.
We currently use ProofPoint for our on-premise Exchange 2016 environment. We use the on premise virtual appliances. It has been in production for about four years now and works great. They also have a cloud offering. There is also Mimecast, which is cloud only and highly rated, but I do not have any direct experience with it.

I registered for SpamTitan. The setup was extremely simple and seems to be working well for the first two hours of operation.
I will run this for the trial period and see how it goes.
I agree with the external / cloud solution and also didn't want a solution where I had to setup each e-mail address externally.
