Outlook: How can I change the letterhead?

Currently my "new" version of outlook creates emails like this:
I would like my emails to look like the old outlook:
In other words, I don't want that DOT with my initials inside.
AND
I want a horizontal line going across the top of the screen.
AND
I want the "To", "From", etc to line up on the left.
