How do I fix 500 error for jQuery?

I have a jQuery error on the following page.

I'm not sure what it is. It works find locally on my Apache and MySQL server. I am attaching the files, along with the database file and a print screen of the error.

The page is a simple database lookup that uses jQuery to send a request each a letter is entered. There are five countries listed
in the database. If you type in f, France will show up on the list. If the country can't be found, it will say, 'No Matches.'
