Exchange 2016 - How are Virtual directories used?

I'm troubleshooting an issue, I think it's related to either the RPC or MAPI vdir. Basically we have 2 sites, primary and DR, we will be performing a failover test tomorrow. In order to test prior, I'm changing my host file to point to the DR site but outlook is not connecting, this is from the outside, not within the Exchange network. It's not the FW's or load balancers, we've confirmed traffic is flowing.

I noticed the Vdir on the servers in the DR site point to a URL that points back to the Primary site.

I'm trying to understand exactly how the vdir (external and internal URL's) work in different scenarios. Any docs or visuals would be appreciated.
